Application Information: 

Please format all proposals according to the following:
1. Summary listing: Name of organization requesting the grant, contact person, brief description of the purpose for the request, mailing address, telephone number and amount requested in 4-6 sentences

2. Cover letter introducing a specific project, if your request is in regard to supporting a project

3. A budget of the project to be considered for funding

4. Organization's mission statement or description of purpose for organization

5. A list of the Board of Directors

6. Copy of the IRS 501(c)(3) determination letter

7. A copy of only the first page of the 990 report for the last two years

We must receive two copies of the above items for each request that is to be considered.

Proposed copies may be stapled or binder clipped. Proposals submitted with covers or folders will not be accepted. Please do not send videotapes, CDs or DVDs. The Fred Maytag Famly Foundation does not distribute an annual report, newsletter or other related materials.
