  1. A brief background of your organization
  2. Description of the project, what it is designed to achieve, and how it will be accomplished.
  3. A proposed budget for the total cost of the project and the amount you intend to request from the trust
  4. A statement describing why participation of the Carver Trust is vital to the success of the project
  5. Indicate history with the trust.

JC Penney Company Fund, Inc.

The fund supports organizations involved with arts and culture, education, health and welfare, cancer, disaster relief, youth development, human services, the retail industry, and civic betterment.
